The health history listed above is correct as far as I know; the above named member has my permission to engage in all program activities, including transportation to and from, at this event except as noted. I understand that adult supervision will be provided. If serious illness or injury develops, medical and/or hospital care will be given. Staff members for the activity are not responsible in case of accidental injury or illness. I further understand that in case of medical emergency I will be notified. In the event that I cannot be reached, I hereby give my permission to the attending physician to hospitalize, secure treatment for, and to order injection, anesthesia or surgery for the child named above.

Medical Waiver
Emergency Medical Treatment Permission
I hereby authorize the 3G camp to obtain, through a physician or other medical professional of its choice, any emergency care that may become reasonably necessary for the participant in the course of camp activities. I guarantee payment of all medical charges for medical treatment. The Sisters of the Order of Saint Benedict will not be liable for any medical expenses.

Photo Release

I hereby grant to the Sisters of the Order of Saint Benedict, St. Joseph, Minn., permission to take photographs of my minor child, for news, advertising and/or promotional purposes in print and electronic media.

I waive any right that I may have to inspect or approve the finished product or products and the advertising copy or other matter that may be used in connection therewith or the use to which it may be applied.

I understand that, although the Sisters of the Order of Saint Benedict will endeavor to use my child’s photograph or likeness in accordance with standards of good judgment, the Sisters cannot warranty or guarantee that any further dissemination of my child’s photograph or likeness will be subject to the Sisters supervision or control. Accordingly, I release the Sisters from any and all liability related to dissemination of my minor child’s, photograph or likeness.

I understand that there will be no monetary compensation for the use of such photo(s) and/or video from the Sisters of the Order of Saint Benedict.

I hereby warrant that I am of legal age, (eighteen [18] years of age or older), and am legally permitted to give this release and authorization on behalf of my minor child. I have read the above authorization and release prior to its execution, and I am fully familiar with the contents hereof. This release shall be binding upon me and my heirs, legal representatives and assigns.

Camp Fee Payment
Camp fee is $300 per child for June session or $350 per child for July session. A deposit fee of $80 must accompany registration. The remaining balance must come by check in one or multiple payments.

Any registrations received after the above dates will be placed on a waiting list. No refunds will be given after these dates with the exception of a COVID-19 diagnosis just prior to camp.
