Nursing Assistant - CNA-  Full Time PM Shift; Part Time PM and Night Shift Positions

Summary: Responsible for providing delegated or assigned home care tasks by RN Case Manager(s) of direct personal care and health-related services to Sister residents.

 

Requirements: 

Certification - CNA Certified Nursing Assistant - MN Registration

Effective interpersonal communication skills, knowledge of direct care services, and ability to fulfill physical demands of the job.

 

Hours: Full Time (40 hrs. per week); Part time 16-24 hrs/wk.

Shifts: Day: 6:00 a.m. -2:30 p.m.; PM Shift 2:00 - 10:30 p.m.; and Night Shift 10:00 p.m.-6:30 a.m. 

Requirements:  Certified Nursing Assistant - MN Registration, work hours include every other weekend and holiday.

RN Case Manager  - RPT-16 hrs/wk., includes On-Call hrs

 

Summary: Provides leadership in coordination of case management services and responsible for assessment of sister resident needs through development and ongoing evaluation of care plans and service plans, care coordination, advocacy, and related case managment services for appropriate treatment to meet the health, safety and well-being of sister residents, ensuring compliance with all applicable standards and regulations. 

 

Qualifications:  Graduate of an accredited school of nursing; Current MN RN License, 5 or more years of experence preferred, advanced nursing related education, knowledge/experience in Class F Assisted Living License preferred, managerial and supervisory experience preferred, sensitivity and aptitude for working with elderly, effective communication skills, and knowledge of community services. 

Hours/Shift: Hours vary based on needs and fill in for RN Staff.  On-call is one week out of every three wks.
